About the Community Wellbeing Hub

In May 2020, we established the Community Wellbeing Hub with Community Connectors who engage with the South Auckland community around their wider health and social needs, ensuring Pacific people have easy access to the services they require in a timely manner. The Hub aims to achieve this through creating an integrated and holistic approach to care.

The Community Wellbeing Hub believes ‘every door is the right door’ to access services and embraces all individuals and families through multiple channels, whether that be their local church, sports club, South Seas or an external provider.

The Hub will act as the connection point for South Auckland primary and community providers to connect individuals and families to the health and social services they need. The service is able to respond to both immediate needs such as food support, essential supplies, income support, access to primary care etc. as well as the longer term goals and aspirations of a family.

Through the establishment of plans of actions, Community Connectors walk alongside individuals and families to support them to achieve their action plan, celebrating milestones and achievements together along the way.

What families can expect

Wellbeing Community Connectors will follow up with any individual or family connected to the Community Wellbeing Hub. The Wellbeing Community Connectors will:

  • Support the client and their family’s urgent needs e.g. food support, essential supplies, income support, access to primary care etc.

  • Provide a warm, in-person handover to support all members of the family to access the health and social services they need.

  • Talanoa with the family to learn about their goals and aspirations, identifying support requirements and a plan of action to achieve these.

  • Provide on-going support and regular check-ins to support the family’s plan of action.

  • Celebrate achievements and milestones together along the way.

When Wellbeing Hub is Available

The Wellbeing Hub is open 9:30am to 4:30pm, Monday to Friday.
